About the Role

As a LPN, you will play a vital role in providing nursing care to patients in their homes, working under the direction of RNs and in accordance with physicians' orders. Your expertise and compassion will make a meaningful impact on the lives of our patients and their families.

Responsibilities

• Provide high-quality nursing care to patients in their homes, adhering to established protocols and guidelines.
• Collaborate with RNs and other healthcare professionals to ensure seamless care coordination.
• Conduct thorough assessments of patients' needs and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Maintain accurate and detailed records of patient interactions and care provided.

Qualifications

• Current licensure as a Licensed Practical Nurse in the state of employment.
• Valid state driver's license and reliable transportation.
• Ability to drive in various weather conditions and maintain a safe driving record.
• Dependable and punctual, with a strong commitment to patient care.
