Job Summary:

The Licensed Vocational Nurse will be responsible for providing direct patient care under the supervision of a Registered Nurse. This includes following the plan of care, providing treatments, and working collaboratively with the healthcare team to achieve positive patient outcomes.

Responsibilities:
  • Assist in identifying patient physical, psychosocial, and environmental needs
  • Provide care utilizing infection control measures that protect both staff and patient
  • Record clinical documentation according to accepted regulatory, corporate, and professional guidelines
  • Participate in interdisciplinary communication to ensure patient needs and expectations are understood
  • Participate in in-service training programs
  • Comply with state regulations and national standards
Requirements:
  • Active LVN license in the State of Texas
  • Ability to react decisively and quickly in emergencies
  • Knowledge of medications effects, indications, and contraindications
  • Experience with multi-tasking
  • Proficiency in using electronic health records (EHR) system
  • Ability to communicate effectively with patients and their family members, and facility staff
Preferred Qualifications:
  • 2 years of experience as an LPN or LVN
  • Familiarity with Medicare and Medicaid insurances and programs
  • Familiarity with home health and hospice
  • Experience in Kinnser/Wellsky (EHR)
