Job Summary

This Behavioral Health Consultant position is part of our primary care treatment team. The successful candidate will work closely with healthcare providers to identify, triage, and manage patients with medical and behavioral health issues. Key responsibilities include providing psycho-education, developing individualized treatment plans, and collaborating with the primary care team to deliver integrated care.

Key Responsibilities:
  • Collaborate with primary care providers to recognize and treat mental disorders and psychosocial problems.
  • Evaluate patient clinical status and triage referrals accordingly.
  • Provide assessment, consultation, and intervention for patients with chronic emotional or health disorders.
  • Work with the primary care team to efficiently and effectively manage chronic health problems.
  • Support primary care providers in diagnosing behavioral health conditions and reduce stigma through education and training.
  • Detect 'at-risk' patients and develop prevention strategies.
  • Evaluate patient care plans with the primary care team.
  • Educate patients, families, and staff about care, prevention, and treatment techniques.
Requirements:
  • Master's degree in Clinical Social Work.
  • Current Licensed Clinical Social Worker (LCSW) issued by the California Board of Behavioral Sciences.
  • Valid California Driver's License and proof of insurance.
  • Basic Life Support (BLS) Certification for Healthcare Providers.
  • Proficiency in Electronic Health/Medical Record systems and Microsoft Office suite.
Preferred Qualifications:
  • Bilingual skills in English and Spanish.
  • At least two years' experience providing psychotherapy (CBT-based models preferred).
  • Experience in health psychology services such as smoking cessation, weight management, stress management, substance abuse disorders, or working with chronically medically ill patients.
  • A general understanding of trauma-informed care approaches, Adverse Childhood Experiences, suicide prevention, and acute crisis assessment/intervention.
  • Familiarity with integrated behavioral health models (IBH).
Compensation & Benefits:
  • Competitive annual salary range: $98,000 - $130,000.
