Job Summary

This position is eligible for the Education Debt Reduction Program, a student loan payment reimbursement program. To be eligible, you must meet specific individual requirements and submit your application within four months of appointment.

Responsibilities
  • Provide medical care in an outpatient setting following PACT principles.
  • Diagnose, treat, and manage a panel of patients.
  • Provide comprehensive patient-centered primary medical care.
  • Function as the Physician Team leader of their Patient Aligned Care Team (PACT).
  • May perform additional duties as a float provider (face-to-face or via telehealth) as assigned.
  • Responsible for providing medical care in an outpatient setting following PACT principles.
  • Will routinely provide medical care using Face-to-Face (F2F), Video Visit (VVC), and/or Telephone appointments.
  • When performing duties as a float provider (F2F or telehealth) for remote patients, will provide care to outreach clinics as needed.
  • Float provider duties (temporary or permanent) may require travel to the supported CBOC(s) periodically to cover a provider vacancy.
  • Assure Veterans receive compassionate, efficient, and safe patient care in their respective care line.
  • Assure clinical care provided meets all timeliness and required documentation as required by VA.
  • Providers must review alerts daily and have no more than 200 alerts at any given time.
  • They must encounter their notes within 72 hours.
  • Encounter errors must be completed within 7 days.
  • Participate in multi-disciplinary team meetings to achieve a patient-centered, holistic approach for clinical care and needs of the Veteran.
  • This includes daily and weekly huddles with your PACT.
  • Comply with all directives and policies related to space management to achieve optimal resource utilization (space, professional, and administration) to maximize clinic utilization within the PACT model.
  • Review available clinical and performance data with the team, focusing on continuous improvement of critical team processes.
  • Participate in PACT performance improvement and sustainment activities to optimize team efficiency and care delivery to veterans.
  • Implement Primary Care operations management processes, as appropriate.
  • Manage communications and facilitate safe transitions of patients between the PACTs site of care and other health care settings, using informal and formal communication methods, as appropriate.
  • Provide health education and health coaching on wellness, disease prevention, chronic care management, and self-management skills to veterans and personal support persons, commensurate with the documented expertise or professional training of the PACT member.
Work Schedule

Monday - Friday from 8:00 AM - 4:30 PM. May vary depending on deployment and/or assigned facility needs.

Requirements
  • United States Citizenship.
  • English Language Proficiency.
  • Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine.
  • Current, full, and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
  • Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
Preferred Experience
  • Minimum of 2 years of PACT experience is preferred, but not required.
Physical Requirements

Must be able to meet physical demands which include strenuous physical activity to include carrying persons and equipment, walking long distances over difficult terrain, running, climbing, working long hours in austere or arduous conditions, long periods of standing, and bending and stooping.

Additional Information

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

Participation in the Coronavirus Disease 2019 (COVID-19) vaccination program is a requirement for all Veterans Health Administration Health Care Personnel (HCP).
