As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with a multidisciplinary team to develop and implement individualized care plans.
Key Responsibilities- Conduct thorough assessments of patients' physical and emotional needs, developing and implementing personalized care plans to meet their unique requirements.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Collaborate with patients' families and caregivers to educate them on care plans, medication management, and home safety.
- Communicate effectively with healthcare providers, insurance companies, and other stakeholders to ensure seamless care coordination.
- Document patient progress, including assessments, interventions, and outcomes, to maintain accurate and comprehensive medical records.
- Active Registered Nurse (RN) License in Colorado.
- Graduate of an accredited school or program of nursing.
- At least 1200 hours of experience as an RN.
- Healthcare CPR/BLS Certification (demonstration course required).
- Home health care experience preferred.
- Excellent organizational and time management skills.
- Able to manage patients with chronic illnesses.
- Pass a background check and drug test (we are required to test for THC).
