As a Hospice Nurse - Public Health and Home Care Specialist, you will be responsible for providing high-quality care to patients in their homes, working collaboratively with our interdisciplinary team to ensure seamless care transitions, and promoting a culture of excellence and compassion within our organization.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and symptom control.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
  • Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and support for patients and their families.
  • Participate in ongoing education and training to stay current on best practices in hospice care and public health.
  • Contribute to a culture of excellence and compassion within our organization, promoting a positive and supportive work environment.

Requirements:

  • Current RN licensure in the state of practice.
  • Minimum of two years of experience in hospice care and public health, with a focus on home care and community-based services.
  • Excellent observation, problem-solving, verbal and written communication skills.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and communication.
  • Valid driver's license and reliable transportation.
