Springwell is seeking a dedicated professional to join our team as a Case Manager. As a key member of our interdisciplinary team, you will play a vital role in ensuring the well-being and independence of our elderly clients.
Key Responsibilities- Conduct Comprehensive Assessments: Evaluate the needs and eligibility of older adults for in-home services, utilizing a comprehensive assessment framework.
- Develop and Implement Care Plans: Create personalized care plans, coordinating services with a network of vendor agencies to meet the unique needs of each client.
- Communicate Effectively: Collaborate with family members, Springwell staff, and community professionals to ensure seamless communication and coordination of care.
- Manage Client Records: Accurately document assessments and care plans into an electronic statewide database, maintaining confidentiality and adhering to regulatory requirements.
- Education and Experience: Bachelor's Degree or higher in a relevant field, or an Associate's Degree with at least 3 years of experience working with elders or providing case management services.
- Technical Skills: Proficiency in computer systems, data entry, and report generation.
- Personal Qualities: Reliable transportation, a valid driver's license, and a safe driving record.
- Generous Time Off: 3 weeks of vacation in the first year, birthday off, 13 paid holidays, 3 paid personal days, and 15 paid sick days per year.
