Heart Transplant Coordinator RN

We are seeking a highly skilled Registered Nurse to join our team as a Heart Transplant Coordinator. This role will play a critical part in the care and management of patients undergoing heart transplantation.
Key Responsibilities- Monitor patients and families before, during, and after transplantation for physical, emotional, psychosocial, and financial needs.
- Coordinate patient and donor pre-admission work-up and visits, gathering and reviewing pertinent clinical information.
- Collaborate with healthcare providers to establish a plan of care and facilitate continuity of care across departments.
- Coordinate all services for BMT patients and oversee National Marrow Donor Program processes as needed.
- Provide education to patients, families, and donors to assist in understanding of the disease and processes surrounding pre- and post-transplant issues.
- ADN or BSN degree
- Three years recent clinical experience in blood transplant, marrow transplant, oncology, or case management/utilization
- Texas RN License
