Job Title: Home Health Behavioral Health RN

Job Summary:

Principle Choice Home Healthcare is seeking a skilled and compassionate Home Health Behavioral Health RN to provide high-quality care to patients in their homes. As a Home Health Behavioral Health RN, you will work under the supervision of a Director of Clinical Services to deliver comprehensive care that meets the unique needs of each patient.

Responsibilities:

  • Provide direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the Director of Clinical Services.
  • Ensure quality and safe delivery of home health services to patients/clients and families/caregivers.
  • Implement plan of care formulated by physician and Registered Nurse.
  • Provide accurate and timely documentation consistent with plan of care.
  • Assess and provide patient/client and family caregiver education and information pertinent to diagnosis and plan of care.
  • Maintain safe and healthy working environment and use safe work methods and procedures in accordance with company clinical standards.
  • Participate in coordination of home health services with Registered Nurse and clinical management.
  • Assist patient in learning appropriate and executing self-care activities.
  • Use equipment and supplies effectively and efficiently.
  • Documents and reports activity as required.
  • Acts as an effective team member.

Requirements:

  • Completion of state approved training program (generally in the form of a diploma, certificate, or associate degree) AND Associates Degree in Nursing or Bachelor of Science Degree for Nursing.
  • RN license (pass NCLEX-PN exam)
  • 1-2 years of behavioral health experience
  • BLS certification
  • Medical knowledge, patience, compassion, kindness, critical thinking skills, knowledge of infection control practices, stamina, and physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, empathy
