Park Nicollet Community Senior Care is seeking a full-time Geriatric Nurse Practitioner to join our team in the west metro area.
As a member of our team, you will work collaboratively to provide high-quality care to an aging population in various long-term care, TCU, assisted living facilities, and complex care/home-based medicine settings.
The ideal candidate will possess a nurse practitioner master's degree or DNP, geriatric, adult, or family certification, and a MN nursing license. Geriatric experience is preferred.
Key Responsibilities:- Provide comprehensive geriatric care to patients in various settings
- Collaborate with healthcare teams to develop and implement patient care plans
- Conduct thorough assessments and evaluations to identify patient needs
- Develop and implement treatment plans to address patient needs
- Monitor patient progress and adjust treatment plans as needed
