About the RoleWe are seeking a Primary Care Physician to join our team at Conviva Care Centers in Boca Raton, FL.
The successful candidate will be responsible for providing high-quality, patient-centered care to our senior patients, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
The ideal candidate will have a strong passion for delivering exceptional patient care, a commitment to staying up-to-date with the latest medical advancements, and excellent communication and interpersonal skills.

Responsibilities- Provide high-quality, patient-centered care to senior patients
- Conduct wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment
- Stay up-to-date with the latest medical advancements and best practices
- Communicate effectively with patients, families, and healthcare teams
- Participate in quality improvement initiatives and patient safety programs
- MD or DO degree from an accredited medical school
- Current medical licensure in the state of Florida
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
