CDTM Pharmacist

The NYC Health + Hospitals is seeking a highly skilled CDTM Pharmacist to join our team in the Ambulatory Care department. As a key member of our healthcare team, the successful candidate will be responsible for providing exceptional patient care and collaborating with healthcare professionals to optimize medication therapy and improve patient outcomes.
Key Responsibilities- Conduct comprehensive medication reviews and reconcile medication regimens to ensure optimal patient care.
- Collaborate with healthcare professionals to develop and implement individualized care plans that address patient-specific needs and goals.
- Provide education and counseling to patients and caregivers on medication management, adherence, and safety.
- Monitor patient response to therapy and make adjustments as necessary to ensure optimal outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
- Collaborate with the care team to develop and implement strategies to improve patient satisfaction and engagement.
- A valid New York State license and current registration to practice pharmacy.
- Certification to practice Collaborative Drug Therapy Management (CDTM) by the New York State Education Department (NYSED).
- Proficiency with Microsoft Visio, Project, Excel, Word, and standard computer programs.
- Prior experience with EPIC EMR/Willow Ambulatory.
- Advanced interpersonal skills and excellent oral and written communication skills.
- Immunization Certification and Current BLS Certification.
