St. Edmond's Home for Children is a residential care facility dedicated to providing exceptional care for children with intellectual and physical disabilities. We are seeking a skilled and compassionate Per-diem LPN or RN to join our team.
Job SummaryThe successful candidate will be responsible for planning, organizing, and executing the Nursing Plan of Care for each resident. This will involve collaborating with the Supervisor and Therapy team to integrate the Nursing/Therapeutic plan of care. The candidate will also be responsible for delegating aspects of the plan to Direct Care staff and maintaining accurate documentation.
Key Responsibilities- Develop and implement individualized Nursing Plans of Care for each resident.
- Collaborate with the Supervisor and Therapy team to ensure seamless integration of the Nursing/Therapeutic plan of care.
- Delegate tasks to Direct Care staff and maintain accurate documentation of resident care.
- Participate in Interdisciplinary Health Planning (IHP) conferences and periodic reviews.
- Evaluate prospective new admissions from a nursing perspective in collaboration with the Director of Nursing.
- Review resident conditions with the Primary Care Physician and Director of Nursing, and implement physician orders, including medication administration, treatments, and diet orders.
- Follow up on diagnostic tests, special studies, and other procedures in conjunction with the Nurse Manager.
- Provide medications, copies of prescriptions, and instructions to family members and staff when a resident is transferred or returns home for extended visits.
- Confer with the Nurse Manager as needed on resident conditions, update the plan of care, and maintain documentation of changes in resident conditions.
- Carry out responsibilities in accordance with St. Edmond's Home policies and procedures, as well as regulatory agency requirements.
- Maintain a current understanding of emergency medical procedures and ensure compliance with policies and guidelines related to emergency medical procedures.
- High School Diploma or GED
- Current LPN or RN licensure in the state of Pennsylvania
- Minimum one year of licensed nursing experience, with pediatric experience preferred
- Knowledge of state and federal regulations, preferably in an ICF/MR facility, is a plus
- Experience working in the field of intellectual disabilities is preferred
- Trach and vent experience is a plus
