Primary Aide

We are seeking a highly motivated and compassionate Primary Aide to join our team at Social Model Recovery Systems. As a Primary Aide, you will play a vital role in supporting our clients in their journey towards recovery. Your primary responsibility will be to assist Primary Counselors in providing services to participants, including screening, intake, assessment, orientation, individual counseling, treatment planning, case management, and documentation.
Key Responsibilities- Model healthy and ethical behaviors in all interactions, adhering to HIPAA and Confidentiality standards.
- Conduct hourly checks on residents to ensure their safety, completing a Night Rounds Checklist if working on a night shift.
- Wake residents at the appropriate time if working a morning shift.
- Administer resident drug tests as directed.
- Provide case management services, including documentation, as needed.
- Facilitate full utilization of service units, including case management documentation, as directed by the supervisor.
- Document services provided according to Agency and regulatory standards, ensuring clarity, concision, and timely completion.
- Assist with resident benefits applications, including DPSS, SSI, and Medi-Cal.
- Monitor resident medications, documenting as applicable.
- Inventory new resident property upon admission, if directed.
- Upload relevant documentation into resident electronic health records, if directed.
- Maintain relationships with family members, community members, and organizations, adhering to regulatory expectations and Agency policies.
- Ensure a safe and alcohol/drug-free environment.
- Perform all other duties as assigned.
Credential required: SUD registered or certified.
