- Monitor patient progress against care plans and ensure timely communication with the healthcare team.
- Develop and implement plans of care, ensuring patient goals are set and met.
- Outcomes Management
- Collect and analyze data on patient outcomes, identifying areas for improvement.
- Develop and implement tools to evaluate and communicate patient progress.
- Leadership and Professional Practice
- Provide input on nursing staff evaluations and competency assessments.
- Act as a consultant to clinical staff, providing guidance on procedures and treatments.
- Maintain clinical competence and disseminate evidence-based practices.
- Education
- Bachelor of Arts/Science degree in a healthcare-related field or BSN preferred.
- Experience
- Two years of clinical experience in focused areas, working with multidisciplinary teams.
- License/Certification
- Current RN or discipline-specific licensure in the state of residence.
