Job Title: Clinical Supervisor, Hospice Intake

VNS Health is seeking a highly skilled and experienced Clinical Supervisor to lead our Hospice Intake team. As a key member of our clinical leadership team, you will be responsible for overseeing the day-to-day operations of our Hospice Liaison Intake process, ensuring the delivery of high-quality patient care and services.

Key Responsibilities:
  • Supervise and coordinate the work of the Hospice Liaison Intake team, ensuring effective triaging of new referrals and admissions.
  • Collaborate with Quality Improvement/Education to develop and implement quality improvement and educational activities to ensure professional practice standards are consistently met.
  • Work effectively with administration to design, implement, evaluate, and modify quality initiatives and educational plans as directed.
  • Communicate effectively with VNS Health Care management and hospice agencies under contractual relationship with VNSNY Hospice Care.
  • Perform all duties inherent in a supervisory role, including staff training, interviewing candidates for employment, evaluating staff performance, and recommending hiring, promotions, and terminations.
Requirements:
  • License and current registration to practice as a Registered Professional Nurse in the state of New York.
  • Valid driver's license or NYS Non-Driver photo ID card, may be required as determined by operational/regional needs.
  • Bachelor's Degree in Nursing.
  • Minimum of three years of progressive clinical experience in home care, Hospice, or related field.
  • Demonstrated knowledge of the regulatory and reimbursement requirements governing Hospice.
