About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Hospice team as a Hospice Registered Nurse - After Hours. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with terminal illnesses and their families in a home or facility setting.

Key Responsibilities
  • Conduct initial evaluations and re-evaluations of patient nursing needs during each visit
  • Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating patient plans of care
  • Complete assessments specific to the provision of Hospice care, including identification of home safety risks and availability of support systems and community resources
  • Provide support at the time of death and perform bereavement assessments as requested
  • Communicate effectively with patients, families, physicians, and other healthcare professionals
Requirements
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum one (1) year experience as a nurse in a clinical care setting
  • Nursing skills as defined by accepted nursing standards
  • Ability to assess and respond to patient needs in varied settings
Preferred Qualifications
  • BSN preferred
  • Experience as a nurse in a Hospice or Home Health environment preferred
  • Familiarity with Home Care Home Base (HCHB)
