Job Summary

The Pediatric Primary Care Social Worker will play a vital role in providing care management services to high-risk pediatric patients and their families/caregivers. This position requires a proactive approach to identify and address psychosocial issues that may impede general medical care and health-related outcomes.

Key Responsibilities
  • Utilize a comprehensive risk stratification algorithm to identify high-risk patients who may benefit from care management services.
  • Collaborate with the inter-professional team to provide care management support for patients with behavioral health concerns and/or chronic health conditions.
  • Conduct interviews with pediatric patients, families, and/or caregivers to assess personal and social adjustment and determine necessary interventions to mitigate risk.
  • Deliver interventions that address barriers to the delivery of general primary care, including connectivity with appropriate resources.
  • Prepare psychosocial assessment histories and develop individualized therapy/counseling plans based on screening, assessment, and goals.
  • Document progress in the medical record, including psychosocial assessment and interventions.
  • Refer patients and family members/caregivers to community agencies for counseling and facilitate connectivity with needed services.
  • Participate in and facilitate team conferences, as appropriate, with pediatric patients, families, caregivers, and community service providers.
Requirements
  • Master's Degree in Social Work
  • Exceptional problem-solving, critical thinking, organizational, interpersonal, and written/verbal communication skills
  • Ability to work in a self-directed environment with attention to detail, follow through, and high level of computer literacy
  • History of positive rapport with patients, families, physicians, and interdisciplinary team
Preferred Qualifications
  • Experience in a medical or other multi-disciplinary agency is desirable, and experience that matches the assigned practice specialty is preferred
Working Conditions

This position requires the ability to stand for prolonged periods of time and work effectively in a fast-paced environment that is subject to changing priorities and diverse social work needs.
