We are seeking a skilled Home Health Aide to join our team at Interim HealthCare - Dubois, PA. As a Home Health Aide, you will provide hands-on personal care and support to patients in their homes, working closely with our interdisciplinary team to deliver high-quality care.

Key Responsibilities
  • Provide personal care and support to patients, including bathing, grooming, and toileting
  • Assist with ambulation, transfers, and range of motion exercises
  • Assist with incidental and instrumental activities of daily living, such as shopping and meal preparation
  • Monitor and report changes in patient condition to supervisor
  • Document observations, activities, and services provided in a timely and accurate manner
Requirements
  • Eighteen (18) years of age or older
  • Six (6) months of experience in healthcare or completion of our preceptor program
  • Able to effectively communicate in English with all levels of the workforce
  • Able to perform and prioritize multiple tasks or functions
  • Able to provide proof of valid driver's license and auto liability insurance, if applicable
Working Conditions
  • Able to constantly travel within our geographic area
  • Meets applicable health requirements to provide patient care
  • Work is normally performed in patient's home
  • Occasional exposure to communicable diseases and other potentially infectious materials
Knowledge, Skills & Abilities
  • Two (2) positive references
  • Evidence of passing a home health aide written knowledge exam
  • Pass state required criminal and abuse background checks
  • Able to effectively communicate and prioritize multiple tasks
