Key Responsibilities- Provide care for patients enrolled in an IDPH-approved Mobile Integrated Healthcare Program.
- Collaborate with nursing and case management staff for pre-planning and patient rounding on identified patients.
- Complete home safety evaluations as required per the MIHP.
- Coordinate compliance and wellness checks with patients and caregivers.
- Review medication compliance and sort medications during patient visits.
- Evaluate caregiver involvement and assess for referrals to other outpatient care providers.
- Provide ongoing patient education and interventions as dictated in the discharge care plan.
- Coordinate care and report to discharge planners, home health agencies, or primary care providers as required.
- Compile community resources available for patients.
- Complete required documentation and reviews community assessment tools with managers.
- EMT-Paramedic and BLS for Healthcare Providers certification.
- High School diploma or GED.
- 4-5 years of clinical/field experience at the Paramedic level.
- Competency in EMS procedures and knowledge as defined by the EMS system.
- Competent patient assessment skills.
- Ability to work collaboratively as a member of the healthcare team.
- Good communication and social skills.
- Empathy.
