Key Responsibilities

As a vital member of the North Area Community Services Team, you will:

  1. Provide crisis intervention, assessment, and case management support to residents facing housing challenges.
  2. Conduct vulnerability assessments for individuals and families experiencing homelessness, adhering to established standards.
  3. Oversee data management for Community Services Programs and assist in training and data accuracy.
  4. Perform additional duties as assigned within the scope of this role.

Essential Duties

Your role will involve:

  1. Supporting Community Services Programs by assisting with casework for families and individuals in need.
  2. Evaluating individuals and families at risk of homelessness for housing services and entering eligible clients into the Coordinated Entry System.
  3. Facilitating educational workshops for community members and clients.
  4. Implementing a variety of support services tailored to client needs, including transportation and resource linkage.
  5. Providing case management and referrals for clients with mental health, emotional, or substance abuse challenges.
  6. Collaborating with case managers and counselors to coordinate services for participants in the SHIFT Program.
  7. Ensuring compliance with educational laws for homeless students and facilitating access to necessary services.
  8. Completing required documentation for services rendered or referrals made.
  9. Reviewing client records to ensure compliance with HUD and other regulatory standards.
  10. Managing client satisfaction surveys for the Community Services North Division.
  11. Conducting intake assessments for client eligibility for various assistance programs, consulting with case managers as needed.
  12. Working collaboratively with leadership and staff to address the needs of families in transitional and permanent supportive housing.

Qualifications

  1. A Bachelor's degree in a relevant field is required.
  2. At least one year of experience in human services is necessary.
  3. Experience working with families in crisis is preferred.
  4. Ability to obtain and maintain SOAR credentials.
  5. Flexibility to work varied hours as needed.
  6. Commitment to program operational boundaries.
  7. Proven ability to work effectively with diverse populations.
  8. Strong communication skills, both verbal and written.
  9. Successful completion of background checks and required testing.
  10. A valid driver's license and reliable transportation are required.
