Essential Job Functions:
  • Verify resident identification to ensure accurate care delivery.
  • Recognize and respond to resident needs and team member requests.
  • Document resident condition changes promptly.
  • Promote resident independence in decision-making.
  • Report resident intake on meal percentage sheets.
  • Document accidents and incidents.
  • Assist with resident move-ins, transfers, and move-outs.
  • Review work shift assignments and schedules.
  • Ensure task completion in accordance with community policies and procedures.
  • Notify Health and Wellness Director of pending absences or tardiness.
  • Report and document resident complaints and grievances.
  • Report resident departures.
  • Document co-worker interactions to meet resident needs.
Additional Responsibilities:
  • Create a positive atmosphere and calm environment.
  • Meet with personnel to identify and correct problems or improve services.
  • Assist team members with personal care functions and explain procedures to residents.
  • Observe and report pressure areas to prevent decubiti and provide skin care.
  • Ensure call system availability and prompt response.
  • Measure and record vital signs.
  • Report resident abuse immediately.
  • Observe, monitor, and report symptoms of potential skin breakdown and decubitus ulcers.
  • Assist CNAs with sling application and other tasks.
  • Attend and participate in orientation programs, training, and educational classes.
  • Follow established safety precautions and report unsafe conditions.
  • Follow smoking regulations and report violations.
  • Wash hands before and after resident services.
  • Review Resident Assessment and Plans of Care.
  • Maintain confidentiality and privacy of resident care and documentation.
  • Report communicable or infectious diseases.
  • Follow universal precautions and isolation procedures.
  • Perform emergency procedures such as CPR and first aid.
Other Important Functions:
  • Document observed data on flow sheets, notes, charts, etc.
  • Introduce new residents to roommates and other residents.
  • Co-workers as appropriate.
  • Watch for and report changes in room temperature, ventilation, lighting, etc.
  • Check light bulbs and report those needing to be changed.
  • Monitor resident personal articles and store them properly.
  • Inventory and store resident personal possessions.
