Community Wellness Nurse Specialist

About the Position:
This full-time RN Community Health Nurse position is available at Saint Elizabeth Community, a non-profit organization dedicated to providing quality care to older adults and people with physical disabilities.
Key Responsibilities:
- Develop and implement individualized wellness plans for SASH participants
- Collaborate with healthcare providers to ensure seamless care coordination
- Conduct home visits and assessments to monitor participant progress
Requirements:
- Degree in Nursing (RN) from an accredited institution
- Valid RN licensure in Rhode Island
- Experience working with older adults and individuals with physical disabilities
- Excellent communication and interpersonal skills
Compensation and Benefits:
The estimated annual salary for this position is $70,000 - $90,000, based on experience and qualifications. Our comprehensive benefits package includes health insurance, a 403B plan, tuition assistance, and career advancement opportunities.
