RN Patient Care Manager

Gentiva Hospice is seeking a highly skilled RN Patient Care Manager to oversee the day-to-day operations of our hospice sites. This is a critical role that requires strong clinical and leadership skills to ensure the provision of high-quality care to our patients.
Key Responsibilities- Manage the clinical and administrative aspects of our hospice program
- Oversee the provision of care to patients and their families
- Develop and implement policies and procedures to ensure compliance with regulatory guidelines
- Collaborate with the executive director, administrator, and senior patient care manager to achieve program goals
- Registered Nurse with a minimum of Associate's Degree in Nursing required
- Three years' experience in direct patient care in a clinical setting required
- Minimum one year of management/supervisory experience required
- Current license to practice nursing in the state of requested employment
