Job Title: Registered Dietitian

Job Summary:

The Registered Dietitian is a vital member of the PACE Interdisciplinary Team at Element Care. This role works under the direction of the Nutrition Coordinator and the Site Coordinator to provide comprehensive nutrition education and assessments to our participants.

Key Responsibilities:

  • Assess participants' nutritional status at enrollment, annually, and as needed.
  • Share relevant participant information with the Interdisciplinary Team.
  • Help plan dietary supplements for high-risk participants.
  • Report observations and participant changes to ADH staff clinicians.
  • Participate in family meetings and case conferences as needed.
  • Provide follow-up treatment and teaching as needed.
  • Make home visits as requested.
  • Interact with participants, caregivers, and families in a professional and courteous manner.
  • Interact with other providers and community professionals in a courteous and professional manner.
  • Maintain accurate records and documentation in a timely manner.
  • Ability to pass a fit test and wear a respirator as required.

Additional Responsibilities:

  • Participate in Quality Improvement projects and assessments as needed.
  • Participate in Element Care Committees and communicate relevant information as necessary.
  • Complete required annual Health Stream trainings in a timely manner.
  • Adhere to all policies and procedures and support the Primary Vision Statement.
  • Adhere to the confidentiality policy of Element Care, including HIPAA guidelines.
  • Be flexible to perform other related tasks as reasonably required.

Requirements:

  • Master of Science Degree in Nutrition (preferred)
  • Registered Dietitian through CDR and current state licensure in the Commonwealth of Massachusetts.
  • Two years of experience working in a community setting.
  • One year of experience working with the geriatric population.

  • Ability to convey information clearly and succinctly with all employees.
  • Ability to lead independently and work well with others.
  • Ability to work within a team concept and be considerate of co-workers and colleagues.
