Responsibilities:- Provide direct care to residents, including administering medications, wound care, and vital sign monitoring.
- Conduct resident assessments, develop care plans, and implement interventions to meet individual needs.
- Collaborate with the interdisciplinary team to ensure seamless care transitions and resident satisfaction.
- Participate in quality improvement initiatives and contribute to a culture of excellence.
- Current licensure as a Licensed Practical Nurse (LPN) in the state of Virginia.
- Minimum of two years of experience in long-term care, preferably in an assisted living setting.
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and colleagues.
- Ability to work a variety of shifts, including weekends and holidays.
- Competitive hourly rate and shift differential.
