Job Overview:
The LPN Medication Care Manager is responsible for providing exceptional quality care and services by administering medications and treatments in a safe and organized manner. This role requires the highest degree of attention to detail and a commitment to ensuring the well-being of our residents.
Main Responsibilities:
- Administer medications and treatments to residents in a safe and timely manner.
- Document medication administration and any pertinent resident information.
- Collaborate with the Resident Care Director and Wellness Nurse to ensure accurate and up-to-date medication information.
- Conduct Service and Health Updates as directed by the Resident Care Director.
- Participate in the development and implementation of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs).
- Transcribe orders and maintain accurate records.
Requirements:
- LPN, LVN, or state-specific licensed nurse credential.
- Certifications in CPR and First Aid.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
- Previous experience working with seniors preferred.
