Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Luther Crest, a Diakon Lutheran Senior Living Community. As a LPN, you will provide individualized care to residents, evaluate their condition, and maintain their electronic health records.

Responsibilities
  • Provide high-quality care to residents according to their care plans
  • Evaluate resident conditions, gather data, and analyze information to develop care plans
  • Maintain accurate and up-to-date electronic health records
Requirements
  • Current State license to practice as a Licensed Practical Nurse
  • Minimum one year of experience in an acute care, rehabilitation, or post-acute skilled care setting
  • Ability to sit, stand, lift, bend, reach, push, and pull items weighing up to 50 lbs.
