About the Role

As a Clinical Care Specialist with Abode Hospice and Home Health, you will play a vital role in delivering exceptional patient care in the comfort of their own homes. This position requires a skilled and compassionate Registered Nurse to provide direct care and coordinate with the interdisciplinary team to ensure optimal patient outcomes.

Key Responsibilities

Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans

Initiate and reevaluate patient plans of care as necessary, utilizing health assessment data to determine nursing diagnoses

Administer medication and treatment as prescribed by the physician in the plan of care, ensuring compliance with all state and federal regulations

Counsel patients and families in meeting nursing and related needs, promoting education and empowerment

Maintain accurate and timely clinical notes, communicating effectively with the physician and interdisciplinary team

Requirements

Graduate of an accredited nursing school with current licensure to practice in the state of operation

1 year of nursing experience required, with 3+ years preferred

Experience in home health or a similar setting preferred, with a commitment to clinical and documentation excellence
