Job Description

The Direct Support Professional plays a vital role at Homes of Opportunity, providing essential support to individuals in our care. Key responsibilities include:

Essential Functions:

  • Providing intervention techniques and support to individuals in a clinical program, transitional, or long-term residential setting.
  • Ensuring the optimal health and safety of individuals served, while assisting them to become integrated into their community and/or the least restrictive environment.
  • Supporting individuals in areas such as mobility, activities of daily living, communication, cognition, health management, social networking, vocational, leisure time management, and community integration.

Responsibilities:

  • Regular and predictable attendance is an essential requirement of this position.
  • Demonstrating professional behavior consistent with Homes of Opportunity's culture, mission, values, policies, and procedures.
  • Responding to signs of abuse, neglect, or exploitation, and following procedures to protect individuals from harm.
  • Implementing Hope Network Safety Plans and Procedures to ensure the safety of individuals served.
  • Knowledgeable of and successfully implementing a Person-Centered Approach/Individualized Service Plan for each individual served.
  • Using person-first language and effective communication skills to establish a collaborative therapeutic relationship with individuals served and professional relationships with their social support network and external stakeholders.
  • Promoting the health and wellness of each individual served in areas such as physical, spiritual, emotional, and social well-being.
  • Understanding formal and informal assessment practices to respond to the acute and/or chronic medical, behavioral, leisure, and/or care needs, desires, and interests of individuals served.
  • Assisting with information gathering related to individual outcome measures and reporting any medical, behavioral, social, or cognitive concerns to the appropriate team member.
  • Documenting routine and any unexpected/unusual incidents related to the care of individuals served.

Educational and Talent Requirements:

  • High school diploma or GED, preferred.
  • Valid driver's license, required.
  • Ability to become certified in CPR/First Aid.
  • Ability to work scheduled shifts, including weekends.
  • Ability to lift 50 lbs non-stationary weight and safely transfer individuals served.
  • Ability to demonstrate empathy, attention to detail, critical thinking/problem-solving skills, and initiative in responding to service/treatment concerns.

Work Experience Requirements:

  • Basic computer skills, required.
