We are seeking a compassionate and skilled Weekend On Call Hospice Nurse to join our team at AdvisaCare. As a key member of our Hospice Family, you will provide high-quality patient care and support to patients and their families in our service area.

Key Responsibilities
  • Triage patient calls and respond to patient needs in a timely and compassionate manner
  • Make patient visits and provide emotional support and guidance to patients and their families
  • Perform tuck-ins and admissions as needed to ensure seamless patient care
  • Collaborate with our interdisciplinary team to develop and implement patient care plans
  • Participate in field visits and office meetings to stay up-to-date on patient status and needs
Requirements
  • Current Michigan RN License in good standing
  • 1 year hospice experience preferred, but not required
  • Empathetic approach to patients and caregivers
  • Ability to adapt to patient needs and status on a weekend on-call basis
