Key Responsibilities- Develop and implement individualized physical therapy treatment plans to help patients achieve their maximum level of physical function.
- Conduct thorough assessments of patients' physical strengths and abilities to inform treatment plans.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive care and effective communication.
- Supervise and evaluate the performance of Physical Therapy Assistants and Home Health Aides to ensure high-quality patient care.
- Accurately document patients' care observations, interventions, and evaluations to maintain accurate records.
- Master's degree from an accredited Physical Therapy Program.
- Minimum of one year of experience in physical therapy, preferably in a home health setting.
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Strong organizational and communication skills.
