The Case Manager (CM) plays a crucial role in coordinating patient services through an interdisciplinary approach. This involves assessing patients to determine the appropriateness of admission, hospitalization, and level of care. Discharge planning begins at admission or prior to it, with ongoing reassessment throughout the hospital stay. The CM monitors quality and risk management issues, reporting as necessary. CMs may be assigned to various units within the hospital or Emergency Department.

Key Responsibilities:
* Communicate effectively with staff, physicians, patients, and families
* Coordinate patient care processes to achieve desired quality outcomes and control resource utilization
* Facilitate patient and family education, promoting continuity of care for optimal patient outcomes. Ensure patient rights by offering choices when appropriate
* Review patient plans of care with the multi-disciplinary team, facilitating and participating in care conferences for complex patients. Communicate interventions and continuity of care in medical records and verbally with the team
* Collaborate daily with patient care staff to assess, evaluate, and plan continuum care, ensuring timely delivery of care. Implement nursing interventions as indicated by the team plan, enhancing and complementing the skill level of nursing staff
* Function independently, utilizing time constructively and organizing assignments for maximum productivity. Arrange schedules to facilitate meetings with physicians for patient care rounds, team meetings, and other opportunities to enhance communication
* Initial assessments are required for all inpatients within 36 hours of admission (average caseload of 12 patients per day)
* Excess days per patient discharge should remain under 1.39 days per contract requirements
* A discharge summary is required for all patient discharges
* Individual Expectation: Employees must document daily interventions on all patients
* Operation Expectation: Employees are responsible for leading daily Interdisciplinary Rounds for their patients on specific medical units to drive timely discharges and plans of care
* CMS Compliance: All Medicare and Medicare Advantage patients require delivery notices for MOON & IMM documentation. Patients also require choice for discharge dispositions

Salary: $58,300 - $114,300 per year (based on location and experience)

Requirements:
* Associate's Degree (or higher)
* Valid unrestricted RN License in Colorado OR Master of Social Work (MSW) Degree
* 5 shifts in a pay period with one day being a weekend day

Preferred Qualifications:
* Bachelor of Science in Nursing (BSN)
* Case Management Certification
* BLS Certification
* 3+ years of recent clinical or case management experience in area of specialization
* Knowledge/understanding of community resources, policies, and procedures
