Job Summary

This is a full-time position as a Patient Care Coordinator RN Lead. As a valued member of our team, you will play a key role in coordinating patient services through an interdisciplinary approach, ensuring optimal quality outcomes and effective resource utilization.

Responsibilities

  • Assessment and Planning
    • Conduct comprehensive assessments to determine appropriateness of admission, continued hospitalization, and level of care
    • Develop and implement individualized discharge plans, reassessed ongoing throughout hospitalization
  • Quality and Risk Management
    • Monitor and report quality and risk management issues to ensure high-quality patient care
  • Clinical Expertise and Communication
    • Utilize clinical expertise to communicate effectively with staff, physicians, patients, and families
  • Team Collaboration and Education
    • Work collaboratively with a multidisciplinary team to facilitate patient education, promote continuity of care, and achieve optimal patient outcomes

Requirements

  • Licensure and Education
    • Valid unrestricted RN License in the state of Colorado or Master of Social Work (MSW) Degree
    • Bachelor of Science in Nursing (BSN)
  • Skills and Knowledge
    • Knowledge/understanding of community resources, policies, and procedures

What We Offer

The salary range for this position is $58,300 to $114,300 per year, depending on experience.
