LPN Homecare Specialist

Maxim Healthcare Services is seeking a skilled LPN to provide one-on-one care to an adult patient in a home setting in the Chicopee, MA area.

We offer flexible day and evening shifts, and no previous homecare experience is required. Our training programs provide caregivers with the skills and confidence to work comfortably and confidently in the homecare setting.

Responsibilities:

  • Utilize the nursing process to assess, plan, implement, and evaluate patient care.
  • Perform focused interviews to identify specific patient needs.
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
  • Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses.
  • Other Licensed Practical Nurse (LPN) or Registered Nurse (RN) duties as assigned.

Qualifications:

  • Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) License in the state in which the Licensed Practical Nurse (LPN) practices.
  • Current Health Certificate (per facility).
  • Current PPD or Chest X-Ray.
  • Current BLS card.
