Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will be responsible for creating, planning, and directing the hospice care team to provide high-quality care to our patients.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Collaborate with the interdisciplinary team to ensure seamless care delivery
  • Provide education and support to patients and their families
  • Manage patient records and communicate with healthcare providers
  • Participate in quality improvement initiatives to enhance patient outcomes

Requirements
  • Current RN license in the State of Georgia
  • 1-2 years of nursing experience (hospice or related experience preferred)
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
