Job Details

Education and Experience Required:

• Must be licensed as an Occupational Therapist in the state of Minnesota.

• Must obtain CPR certification within 6 months from date of hire.

• Must have a valid driver's license and use of an insured personal vehicle.

• Must complete and pass a state-required background study.

• Able to communicate verbally and in writing to the extent required by the position.

• Able to physically perform the duties required by the position.

• Required to have the ability to read, write, and speak English well enough to communicate with staff, clients, and visitors.

Key Responsibilities

Direct Client Care:

• Evaluate clients; obtain and interpret subjective and objective data.

• Develop treatment plans and programs, including short-term and long-term goals in accordance with physician orders.

• Provide quality care that assists clients to meet goals; complete re-evaluations per requirements.

Client Records and Billing:

• Provide accurate and timely documentation of services rendered utilizing agency EMR.

• Demonstrate understanding and knowledge of all payer sources and authorization required.

• Complete and document supervisory visits with the PTA and/or HHA per regulations.

Communication:

• Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team.

• Demonstrate exceptional verbal and written communication skills.

Follow-Up Programs:

• Recommend and ensure continuation of appropriate therapeutic follow-up programs and activities.

• Develop programs to optimize successful discharge with the highest function possible.

• Make referrals to continuation programs as necessary.
