Job Title: Advanced Practice Clinician
Job Summary:
We are seeking a highly skilled Advanced Practice Clinician to join our team at AlignMed Partners. As a key member of our healthcare team, you will provide primary and urgent care services to nursing center residents in collaboration with other medical providers.
Responsibilities:
- Provide high-quality patient care to residents in a long-term care setting
- Collaborate with the Center Leadership Team to achieve AlignMed Partners' mission of improving the lives we touch through the delivery of high-quality healthcare and everyday compassion
- Work collaboratively with the Medical Director and/or Attending Physician for patient-specific medical care
- Participate in the development and implementation of care plans for residents
- Conduct regular assessments and evaluations to ensure the highest level of care
Requirements:
- Graduate from a program accredited by the ARC-PA and take the PA National Certifying Exam (PANCE) to use the PAC designation
- PA state licensure required
- NCCPA certification required
- A minimum of two years of experience working with adult or geriatric populations is preferred
