Job Summary:

The Licensed Vocational Nurse (LVN) is a vital member of our healthcare team at Christian Care Centers- Fort Worth. As a skilled nursing care provider, you will be responsible for supervising direct care staff, performing direct care according to current standards of practice, state and federal regulations, and facility policy while overseeing the health and wellness of assigned residents.


Key Responsibilities:
  • Perform focused assessments, interim assessments for residents within scope of practice.
  • Assess/Intervene/Report changes in residents' physical/emotional conditions, reactions to medications and treatments, attend and participate in care conferences, and report to responsible party or physician as needed.
  • Assess/Monitor assigned residents' progress and give feedback on individual interventions with care team.
  • Perform direct resident care requiring nursing skills (as approved by TBNE and within scope of practice); including, but not limited to IV, Wound Vacs, Injections, Gastrostomy Feedings, Ostomy Care, Tracheostomy Care, surgical drains.
  • Carry out/coordinate physician orders in a timely manner.
  • Participate and complete discharge-focused assessments and required patient/family teaching follow-up.
  • Direct/Assign work for direct caregivers (CNA, CMA) to ensure quality care for residents.
  • Communicate clearly and professionally with Families, Residents, Professional Providers, and Teammates.
  • Requisition approved materials, supplies, and equipment to carry out planned/ordered care; which may include medication refills being maintained in a timely manner, checking orders for errors and accuracy.
  • Maintain a safe environment for residents; including adherence to infection control policies.
  • Participate in ongoing training and in-services for improving knowledge and updating skills.
  • Perform other assigned tasks as requested and in accordance with policies and procedures.
  • Establish and encourage an atmosphere of optimism, warmth, and interest in residents' activity preferences and needs.

Requirements:
  • LVN degree from an accredited nursing program.
  • Licensure: LVN active license to practice in the state of Texas.
  • Experience: Minimum of one year in nursing experience in long-term or other health care setting. 2+ preferred.
  • Skills: Assessment and clinical skills (med surg level), supervisory and communication skills, excellent customer service skills.
  • Basic computer skills.
  • Current BLS certification.
  • Certified Dementia Practitioner Certification preferred - the course is offered and required once hired.
