Sea Mar Community Health Centers is a leading provider of comprehensive health services to diverse communities. We are seeking a skilled and compassionate Pathways Community Health Worker to join our Care Management team in Everett, WA.

Job Summary:

The Pathways Community Health Worker will work closely with clients, community organizations, and other departments to provide culturally sensitive and geographically connected support. This individual will serve as a link between underserved communities and existing community resources, assisting clients in overcoming barriers to health, social services, education, and employment.

Key Responsibilities:
  • Conduct standardized patient intake and risk assessments
  • Establish trusting relationships with clients and community partners
  • Identify and assist clients with client-identified health and social service goals
  • Assist in developing strategies to address or eliminate barriers to care
  • Work collaboratively with the Program Manager, PCP, and other assigned personnel to ensure all relevant client needs are identified and addressed
Requirements:
  • High School Diploma or Equivalent required
  • Community outreach or case management experience preferred
  • Experience working with people from various backgrounds required
  • Experience working with individuals with substance use disorders, homelessness, chronic mental illness, or people in crisis preferred
