Job Summary:
As a Licensed Practical Nurse (LPN) on our Long Term Care (LTC) team at LifeBridge Health, you will play a vital role in providing high-quality patient care to our residents. Under the supervision of the Registered Nurse/Director of Nursing, you will utilize your nursing skills and knowledge to develop and implement individualized care plans that meet the unique needs of each resident.
Responsibilities:
- Provide direct patient care to residents on the LTC unit, including administering medications, performing vital signs, and providing wound care.
- Conduct thorough assessments of residents' physical and emotional needs, and develop and implement individualized care plans to address these needs.
- Collaborate with the interdisciplinary team to ensure that residents receive comprehensive care that meets their physical, emotional, and social needs.
- Assume leadership responsibilities as needed, including mentoring new staff members and participating in unit-based quality improvement initiatives.
- Stay up-to-date with the latest nursing practices and technologies, and participate in ongoing education and training to enhance your skills and knowledge.
Requirements:
- Graduate of an accredited school of nursing with a diploma or Associate's degree in nursing.
- Current licensure as a Licensed Practical Nurse (LPN) in the state of Maryland.
- AHA CPR certification.
- Basic computer skills, including proficiency in electronic medical records and other healthcare software.
- Ability to work effectively in a fast-paced, dynamic environment and to prioritize tasks and manage time efficiently.
