Affinity Hospice is a leading provider of hospice care, seeking a Licensed Master Social Worker to join our team. As a key member of our interdisciplinary team, you will provide psychosocial support to patients and their families, assessing their needs and developing personalized plans to alleviate problems and provide supportive counseling.

Responsibilities:

* Provide a wide range of psychosocial services to terminally ill patients and their families
* Regularly assess the patient and family psychosocial needs
* Implement the Plan of Care provisions to meet patient needs
* Participate in on-call rotation with other social work team members

Qualifications:

* Master's Degree in Social Work
* Professional licensure as a Master Social Worker in the state of employment
* Clinical experience or formal training related to death, dying, and bereavement
* Knowledgeable of hospice state and federal regulations
* Ability to work a flexible schedule, including evenings and weekends
