Key Responsibilities- Coordinate clinical, psycho-social, and spiritual services for patients through case management
- Collaborate with the interdisciplinary team to meet patient needs and develop effective care plans
- Communicate with patients and families to explain hospice services and Medicare benefits, obtain informed consent, and ensure documentation is accurate
- Perform supervisory duties for Hospice Aides and Licensed Practical/Vocational Nurses, adjusting assignments as needed to promote optimal patient outcomes
- Monitor patient symptoms and respond to changes in condition, collaborating with the interdisciplinary team to develop and implement effective interventions
- Provide physical and emotional support to patients and families during periods of crisis, utilizing case management skills to coordinate care and ensure continuity
- Maintain accurate and comprehensive care plans, reflecting current problems, goals, and interventions for patients
- Stay up-to-date on clinical knowledge and best practices, using evidence-based guidelines to inform nursing care
- Associate or Bachelor's degree in Nursing required
- Minimum of one year of nursing practice or equivalent experience preferred
- Hospice, oncology, home health, and/or long-term care experience preferred
