About the Role

Mercy Health Rehabilitation Hospital is seeking a skilled Case Manager to join our interdisciplinary team. As a Case Manager, you will play a vital role in the care and recovery of our patients, working closely with healthcare professionals to develop and implement individualized treatment plans.

Responsibilities

  • Complete departmental orientation and initial competencies
  • Collaborate with other departmental staff to identify barriers to care and develop solutions
  • Document patient progress and complete the Individual Plan of Care (IPoC) per CMS guidelines
  • Schedule family conferences and communicate with caregivers to keep patients informed of progress
  • Coordinate weekly patient care team conferences to refine treatment plans
  • Review patient assigned CMG and identify potential missed comorbid conditions
  • Perform other duties as assigned

Qualifications

  • Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist
  • Certification in Case Management or Rehabilitation Nursing preferred
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred
  • Effective oral and written communication skills in English, additional languages preferred
  • Basic computer skills in excel, word, outlook, power point, etc. required
