Physician - Post-Acute Care Specialist

We are seeking a highly skilled and dedicated Physician to join our post-acute care team at Adfinitas Health. As a Physician, you will be responsible for managing the health problems and coordinating high-quality care for our patients in skilled nursing facilities, rehabilitation centers, and long-term care facilities.
Key Responsibilities:
- Perform exams, complete assessments, and develop treatment plans for patients
- Conduct History & Physicals on new admissions to skilled nursing facilities
- Adhere to Adfinitas Health policies for patient care and documentation
- Collaborate with multidisciplinary teams to ensure seamless patient care
- Supervise Advanced Practice Providers (APPs) as needed
- Submit accurate and timely charges for professional services
Requirements:
- Board eligible or certified in Internal Medicine or Family Medicine
- Geriatric fellowship training or experience in post-acute care is preferred
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Strong computer skills and proficiency in electronic medical records (EMRs)
