Job Summary

We are seeking a skilled Physical Therapist to join our team at Louisiana Extended Care Hospital of Lafayette, a part of LHC Group. As a Physical Therapist, you will play a vital role in helping patients achieve optimal physical function and independence.

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, consulting with physicians to develop therapy plans of care.
  • Provide physical therapy services within the scope of practice, in accordance with the plan of care and in coordination with the interdisciplinary team.
  • Observe and report patients' responses to treatment and any changes in their condition to the nurse supervisor and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
  • Instruct and inform participating members of the healthcare team, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
  • Document outcomes of evaluation, goals, and anticipated projected frequency of care, as well as complete progress notes in the medical record.
Requirements
  • Graduate of an accredited school of Physical Therapy and successful completion of the National Registry exam.
  • Current Physical Therapy License in the state of Louisiana.
  • CPR certification.
