Job Summary

Full Circle Health Network is seeking a highly skilled CalAIM Program Manager to support the development of core service delivery models for enhanced care management, community health worker, and community supports. This is an exciting opportunity to be at the nexus of California's CalAIM Medi-Cal reforms to help make the systems work better for at-risk or justice-involved youth.

Key Responsibilities:

  • Collaborate with internal and external stakeholders to develop Full Circle's CalAIM service models of care that reflect the lived experience and honor the knowledge of participating CBOs.
  • Represent Full Circle and providers at key meetings to inform and shape ongoing policy development in this space.
  • Oversee the program certification process for new providers.
  • Support the development of outcome metrics and monitoring processes to track program success and identify opportunities for improvement.
  • Work closely with the quality department to ensure providers are implementing strategies that result in accurate data collection and continuous quality improvement toward expected outcomes.

Requirements:

  • Bachelor's degree.
  • 5-7 years of experience developing and implementing Medi-Cal and/or social services programs with a preference for youth programs.
  • Licensed professionals (LVN, RN, LCSW, LMFT) preferred.

Preferred Qualifications:

  • Ability to work independently and maintain positive relationships with internal and external contacts at all levels.
  • Ability to communicate and work effectively with a variety of providers and individuals demonstrating problem-solving skills.
