Job Summary

We are seeking compassionate and skilled Private Duty Nurses (LVN/LPN) to join our team in Houston and surrounding areas. As a Private Duty Nurse, you will provide continuous care to our patients in the comfort of their own homes, helping them transition from hospital to home and ensuring they receive the best possible care.

Responsibilities
  • Implement physicians' prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in our system of record
  • Teach and train family members on health promotion and disease management
  • Accept and write verbal orders from physicians as required
  • Follow up with physicians, clients, family members, and staff on client problems and concerns
Qualifications
  • Active, unencumbered LPN/LVN license in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • Reliable transportation
