The Laurels of Bedford seeks a skilled MDS Nurse to lead clinical coordination efforts and ensure exceptional care for residents.

About the Role

This key position involves assessing resident needs, coordinating with care teams, and maintaining accurate records within regulated timeframes.

Responsibilities:
  • Conduct thorough assessments and develop tailored care plans in accordance with RAI processes.
  • Maintain open communication with caregivers, including physicians, nursing staff, social services, therapy teams, dietary specialists, and activity coordinators.
  • Review information from hospitals, consults, and outside agencies to inform assessment and care planning.
  • Promote resident-centered care by identifying and initiating significant changes to MDS assessments.

In collaboration with the Care Management Coordinator, this role ensures timely completion of PPS meetings, maintains accurate Medicare day tracking, and optimizes RUGs utilization.

Qualifications & Requirements
  • A registered nurse (RN) or licensed practical nurse (LPN) with a strong background in gerontological care.
  • AANAC certification (RAC-CT) is preferred but not mandatory.
  • Experience as an MDS Nurse or equivalent expertise.
  • Familiarity with the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS).
Benefits and Compensation

The successful candidate can expect a competitive salary range of $85,000-$110,000 annually, commensurate with experience, along with an excellent benefits package that includes health insurance, 401(k) matching, paid time off, and paid holidays.
