We are seeking an experienced MDS Nurse to fill the position of MDS Coordinator at The Laurels of Bedford.

The estimated salary for this role is $10,000 per year, plus a $10,000 sign-on bonus.

Job Description:

The MDS Coordinator provides oversight of the RAI process and conducts assessments and care plan coordination for residents. This role requires strong communication and leadership skills, as well as experience working in long-term care settings.

Responsibilities:
  • Completes the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care plans within regulated time frames.
  • Coordinates scheduling the Resident Assessment Instrument (RAI) process with the interdisciplinary team.
  • Assesses residents through physical assessment, interview, and chart review.
  • Discusses resident care needs with caregivers, including physicians, nurses, social services, therapists, dietary staff, and activity staff.
  • Reviews information from hospitals, consultants, and outside agencies, and uses this information in the completion of the assessment and care planning.
  • Coordinates, identifies, and/or initiates significant changes to MDS assessments.
  • Is prepared to conduct Prospective Payment System (PPS) meetings, maintaining MDS assessments per Medicare schedule and monitoring Medicare days and Resource Utilization Groups (RUGs) utilization in the absence of the Care Management Coordinator.
Requirements:
  • RN or LPN licensure
  • AANC certification is preferred, with RAC-CT credential also beneficial.
  • Knowledge of the Resident Assessment Instrument (RAI) process, including principles of the Prospective Payment Process (PPS), is strongly preferred.
  • Experience as an MDS Nurse is essential.
